I must be missing a step. After I have exported my document, I have a folder with a index.html file and an assets folder.

How does that then become a Web app?

I've uploaded those files by FTP and can view them on my phone. But the app icon is missing.

Do I need to then take these files into something like Baker Framework or PhoneGap Build, or is that only if I want to have my apps hosted on the iPhone and/or Google Play stores?

You are correct that packaging with Baker or PhoneGap Build are processes for hosting apps in stores.

From the Help docs: You'll need to Output to a Web App. A custom icon can be set in the Advanced section of the in5 dialog.
